Faulty Equipment Accidents

Five examples of accidents resulting form faulty equipment in the mining sector:

    Faulty Rope Four miners at a Wyee coal mine were sent hurtling down the entrance shaft when a rope on a draft transporter snapped as they headed underground to begin their shift. Three miners jumped clear whilst the fourth rode the run-away transporter 150 metres down the steep slope to the bottom, sustaining minor injuries. The rope was apparently faulty, in a badly frayed condition. Had the rope snapped on the next trip, 50 men would have been in the transporter.

    Faulty Ladder A contract rigger engaged in replacing machinery fan covers on a dragline sustained a fractured pelvis after falling 3 metres from a faulty ladder used to access the top of the fan covers. The ladder was faulty (rubber feet of ladder missing and ground off).

    Faulty Flushing Canister A fitter suffered deep lacerations to his thighs, when an aluminium flushing canister he was using burst. The failure of the canister was due to over pressurising the canister, which was not fitted with a pressure relief valve.

    Falls from Man Baskets There have been a number of incidents in which man baskets have detached and fallen from front end loaders fitted with a quick hitch mechanism and other equipment fitted with man carrying baskets. The cause of these accidents has been a failure of the locking device with the man basket subsequently detaching.

    Faulty power tools A mine maintenance contract fitter, working on the screening level of a processing plant, received an electric shock and severe burns from a 240V double insulated hand held impact power tool that was being prepared for storage after use. The power tool chuck insulation failed due to an extremely dirty tool (internally) with fine dust and grime evident. Any power tool used must be fit for purpose, and maintaining tools in a satisfactory condition for use is essential.
